Work in partnership with others as part of a structured inter-disciplinary team, within in a Certified Community Behavioral Health Center (CCBHC) model, to provide a set of social service functions that helps clients access the resources they need to recover. Responsible for individual, family and/or group services providing high quality targeted case management services that will assist individuals in sustaining recovery, and gaining access to needed medical, social, legal, educational, and other services and supports and monitoring in a culturally sensitive way.

Provide case management (referral, linkage, and advocacy) services and individual and group rehabilitation to clients who have been incarcerated either in prison or jail and are re-entering the community. Case Managers will initially provide assessments in the prisons for referred inmates regarding severe mental illness and suitability for RICCT program, and will establish contacts with the Department of Corrections Discharge Planners and coordinate the re-entry of those inmates accepted into the RICCT program upon release. Case Managers will assist clients with housing, establishing benefits and employment, and will provide group rehab on topics related to mental illness and reintegration in the community. They will provide crisis services as needed and consult with client’s probation/parole officers when appropriate.

Requirements Bachelor’s Degree in the social sciences field is preferred. Consideration may also be given for degrees in Criminal Justice or Education. Case Management Certification or obtain by taking the first available Case Management Certification course provided by ODMHSAS after hire. Must have a strong commitment to the right and ability of each person served to live in normal community residences; and have access to helpful, adequate, competent, and continuous supports and services. Skills and competence to establish supportive trusting relationships with persons with mental illnesses and/or substance abuse issues and respect for client rights and personal preferences in treatment are essential. Must possess a valid Driver License and satisfactory driving record and use personal automobile to travel to locations other than primary office.

Due to the “safety-sensitive” classification, an employee in this position would be subject to drug and alcohol testing, including random testing. Marijuana is one of the substances included in the drug panel screening. Possession of a medical marijuana license will not excuse you from the testing process or the consequences of testing positive for marijuana per the Family & Children’s Services Drug Free Workplace Policy, including possible revocation of a job offer or dismissal from employment.
