Loyal Source Government Services is looking for an experienced Chart Auditor for an opportunity in Pecos, TX.

Lodging and meals will be provided throughout the duration of the contract.

KNOWLEDGE & ABILITIES

• Knowledgeable and familiar with medical terminology
• Previous experience conducting chart audits
• Strong attention to detail and good analytical skills
• Strong written and oral communication skills
• Proficiency with Microsoft office products (i.e. Excel, Outlook, Word) along with the ability learn other operating systems like Vision Link and UAC Portal
• Ability to communicate effectively with providers and nurses to ensure all medical information is accurately captured

RESPONSIBILITIES

• Review charts for completion and accuracy (i.e. HCG, LMP, COVID and communicable diseases, signatures, dates)
• Review charts and ensure referrals are appropriately documented and delivered to the referral department
• Note special healthcare needs and communicate needs with leadership team and quality assurance nurse for reporting purposes
• Enter medical health assessment encounters into UC Portal and Vision Link (two separate databases)
• QA this process to ensure labs, referrals, vaccines are documented in both systems appropriately
• Identify if vitals are WNL; if not ensure they are/were addressed by provider and intervention documented
• Ensure follow up notes are documented in UC Portal
• Ensure ordered medications are transcribed correctly onto MAR
• All other duties as applicable
