The Community Outreach Worker
will be trained as a Parent Educator in the Parents as Teachers program. They will be responsible responsible for establishing and maintaining regular and long term relationships with
families providing family-centered and strength-based interventions. Being non-judgmental and
empathic, assist in strengthening the parent-child interaction and improving parent’s skills to optimize
the home environment and parenting practices that promote the optimal development and health of
the child. Assist with increasing the family support systems and ability to problem solve and advocate
for themselves and their children. Responsible for assisting families in establishing goals and plan for
the accomplishment of these goals. Assessment of normal growth and development of the child.
Identify and refer to other agencies and healthcare services.

Responsibilities:
The day in the life of the The Community Outreach Worker at Hackensack Meridian Health includes :
  • Identify children between 0-6 months of age and pregnant women among high needs families in Union County to enroll them in the Parents as Teachers program.
  • Maintain a case load of twenty families.
  • Establish a trusting relationship to assist parents in strengthening parent-child interaction, optimizing the home environment and parenting practices.
  • In an empathetic and tactful manner, assist the family in creating goals and a plan to accomplish these goals.
  • Using knowledge of normal child growth and development and parent-child relationships, provide curriculum interventions that are family centered and strength based.
  • Maintain relationship with the family for a minimum of two years.
  • Using strong interpersonal skills and the ability to relate to individuals who may not share basic commonality, including value systems and behavior norms, provide education and support to families in case load.
  • Assist in planning and execution of monthly Group Connections meetings.
  • With patience and tact, assist families to increase their support systems and the family’s ability to problem solve and assume the role of advocating for themselves and their children.
  • Maintain a resource network to connect families including healthcare and social service agencies.
  • Establish relationships with community groups, social service agencies, other outreach programs that may assist families.
  • Participate in community activities such as health fairs as it pertains to pre-natal, post-partum and early infancy topics.
  • Works with local high schools to provide support and services as needed.
  • Maintain accurate records of the home visits, assessments and necessary documentation of the Parents as Teachers program, New Jersey Department of Children and Federal Government bench marks.
  • Participate in educational programs and maintains records of attendance in accordance with Parents as Teachers certification.
  • Participate in weekly staff meetings.
  • Complete required documentation on Home Visits and provides to supervisor on a weekly basis for review.
  • Perform other duties as requested.
  • Other duties and/or projects as assigned.
  • Adheres to HMH Organizational competencies and standards of behavior.
Qualifications:
Knowledge, Education and Skills Required:
  • Associate Degree necessary, but Bachelors degree is preferred. Degree in Early Childhood, Children and Families Services or related field.
  • Fluent in Spanish.
  • Parents as Teachers certification( will provide during employment)
  • Maturity and experience in working with children and their families for at least two years.
  • Working with culturally diverse communities/families and the ability to be culturally sensitive.
  • Ability to establish and maintain personal/programmatic boundaries
  • Computer literate
