Job Title: Direct Care
Day: Weekends/Weekdays - 5 days work, 2 days off
Please indicate which shift:
Swing Shift - 3:00 pm - 11:30 pm
NOC/Graveyard Shift - 11:00 pm - 7:30 am
Please note:
- An individual must obtain a criminal record clearance (via Live Scan) to work, reside in or volunteer in a licensed facility. If the individual has been convicted of a crime other than a minor traffic violation, the individual cannot work or be present in any community care facility.
- Must have worked with at-risk youth
- Must have experience with de-escalation and compassionate care
Job Summary: The Direct Care team member is responsible for performing a comprehensive set of routine activities that support the rehabilitation and safety of residents. The Direct Care team member is tasked with monitoring and supervising residents to ensure a safe environment throughout the day and/or night.
Work Environment: The Teen Project Inc., operates Sanctuary Recovery Center (SRC) Short-Term Residential Therapeutic Program (STRTP), serving foster youth experiencing substance use disorder who are victims of commercial sexual exploitation, gang affiliation, frequent runaways, and wards of the state in Orange County. SRC is an integrated program of specialized and intensive care, supervision, services, support, and substance use treatment in a structured and therapeutic environment. Services are provided through the support of Orange County Social Services Agency, gracious donors, and other applicable funding sources. SRC aids as an innovative center that combines, substance use disorder treatment, mental health treatment, education support, housing, and workforce development.
Minimum Qualifications:
- RADT, registered drug and alcohol technician, with CCAPP, or similar Accreditor
- 1+ year experience working in youth related setting
- Valid California driver’s license and proof of automobile insurance
- Bachelor's Degree
- Must be at least 21 years of age
- Proficient in Windows and Microsoft Suite (Outlook, Word, Excel)
- First Aid/CPR Certified
Upon Hire:
- Tuberculosis clearance
- Physical clearance
- Pre-employment drug screening
- Live Scan/background check clearance
Duties and Responsibilities:
- Supervise residents within their day to day activities
- Act as a mentor and role model by teaching independent living skills, behavior modifications, and crisis management
- Monitor resident chores
- Monitor self‐administration of medication
- Gather and ensure accuracy and completeness of intake and enrollment process
- Complete MAR forms for every resident upon enrollment, monthly, and as needed (i.e. medication refills)
- Assist residents with homework and/or other appropriate projects of interest to the resident; serve as residents’ educational liaison
- Schedule resident doctor/dental appointments
- Transport residents to school, activities, and appointments
- Perform and document rounds at 15-minute intervals making accurate note of the resident’s location and any other relevant information
- Monitor and facilitate groups (i.e. attendance, timeliness, behavior etc.)
- Read electronic shift reports
- Present and discuss shift change reports
- Conduct facility walkthrough regularly
- Maintain cleanliness and orderliness of the facility
- Monitoring of basic laundry services, including but not limited to washing, drying, and ironing
- Complete facility inspection checklist
- Report maintenance related issues
- Monitor and report resident behavior
- Conduct and coordinate visual check, persons search, and/or property search upon resident entry and re-entry
- Submit Incident Reports as needed
- Lock and alarm facility entrance/exit doors
- Attend designated staff meetings and any offsite meetings or trainings that may be established
- Maintain quality, confidentiality, and compliancy of all documentation and services within designated timeframes
- Ensure the organization functions in compliance with the standards of the organization, HIPAA, Orange County Social Services Agency, and The Joint Commission
- Perform supplemental duties assigned by the management
