Director, Care Coordination for Christian Hospital
Overview
Christian Hospital is a full-service acute care community hospital located on 28 acres in north St. Louis County. Christian Hospital is noted for its excellence in heart services and lifesaving cardiothoracic surgery, emergency medicine, breast health services, orthopedic/spine surgery, hyperbaric wound care, pain management, vascular surgery, endocrinology, gastrointestinal care, stroke care, nephrology, therapy, radiology, urology, pulmonary care and treating sleep disorders.
Northwest HealthCare, located six miles west of Christian Hospital, provides the community 24-hour emergency care, with Washington University Physicians and St. Louis Children’s Hospital providing pediatric services. Northwest HealthCare is also home to the Sleep Disorders Center and a variety of full-service outpatient diagnostic and imaging services in a convenient setting.
The Case Management Department provides services through a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet an individual's needs. The Case Management Department is responsible for all acute care inpatients where needs have been identified or who have requested assistance, emergency department patients where needs have been identified, and community individuals who request assistance.

Role Purpose
Selects, orients, monitors, assigns projects, counsels, problem solves, leads employees under his/her jurisdiction. Practices in accordance with Missouri Nurse Practice Act and HSO policies and standards of care. Leads and directs the coordination of patient care at the most appropriate location, level and time.
Responsibilities
- Manages individual(s) including but not limited to: onboarding and hiring, training, workload management & performance evaluations, conducting professional development plans as needed. Ensures that the productivity and actions of that group meet/support the overall operational goals of the department as established by department leadership.
- Develops and adheres to departmental staffing, revenue and/or expense budgets. Responds to changes in the business which may affect the ability to achieve the budget goals.
- Monitors, controls and evaluates all resources utilized in Case Management, Social Services and Congestive Heart Failure Disease Management Departments.
- Directs the extension of care coordination in the inpatient setting.
- Provides designated service lines and physicians timely data, assists in analysis of the data and identifies areas for administrative focus.
- Oversees the care of patients referred to the Hospitalist Service to ensure timely hospital discharge and continuous communication with Primary Care Physicians, patients and families, specialists, hospital departments and administration.
