Job Title: HCS Case Manager
Reports to: HCS Director
FLSA Status: Exempt
Department: HCS

Summary: Directs agency or major function of public or voluntary organization providing services in social welfare field to individuals, groups, or community by performing the following duties personally or through subordinate supervisors.

Essential Duties and Responsibilities include the following including other duties as assigned:

  • Manages at a minimum 15-25 HCS participants medically- Supported Home Living.
  • Manages activities of the HCS Program Manager to ensure assignments and deadlines are met.
  • Meets weekly with the Chief Operations Officer.
  • Meets weekly with the HCS Program Manager.
  • Oversees, manages and provides guidance and support to ensure group home is functioning at top quality.
  • Oversees HCS program and provides Client Coordination in compliance with the definition in the HCS Service Definitions and Billing Guidelines for individuals receiving HCS funding.
  • Assists Chief Operations Officer in implementing teaching and training philosophy emphasizing improved, independent functioning for each individual.
  • Ensures each individual’s humanity and dignity is respected.
  • Ensures the rights of the individual or the individual’s rights as exercised by the LAR on behalf of the individual are protected.
  • Ensures that individuals, the individual’s LAR and family members, with the consent of the individual or the individual’s LAR, participates in making choices about where the individual will live, attend school, work and take part in leisure activities.
  • Coordinates the development and implementation of the individual’s PDP and justifications of service.
  • Provides oversight of all participant’s IPC, monthly reviews of all utilization reports and ensures all increases necessary for client care are coordinated in conjunction with the client’s IPC needs.
  • Integrates various aspects of services delivered under the HCS Program and other sources.
  • Records each individual’s progress or lack of progress.
  • Performs an audit on 12 random charts monthly and assures corrections are made.
  • Performs billing of all HCS services and reports IPC, funding and all Care System/Billing errors to the COO and CFO.
  • Assists the COO with the HCS Program CAP, HCS Provider Fairs, client enrollment, and County contracts.
  • Works with all Service Coordinators from the local authority for individuals with intellectual disabilities to provide continuity of care and ensure that client’s receive services that reflect their PDP and individual choices.
  • Prepares and participates in HCS Annual Audits.
  • Ensures HIPPA standards are followed at all times and client confidentiality is kept.
  • Follows Texas Department of Family Protective Services and Adult Protective Services guidelines and regulations regarding Abuse, Neglect and Exploitation.
  • Works with all Service Coordinators from the local authority for individuals with intellectual disabilities to provide continuity of care and ensures clients receive services that reflect their PDP and individual choice.
  • Ensures that all family grievances are reported to the COO.
  • Tracks changes in HCS rules and regulations and notifies Program Manager, Client Coordinators, Nursing Department, CEO and COO of changes.
  • Writes contracts with OT, PT, Speech, Dietitians and any other outside service providers. (Gets COO to sign and approve new contracts)
  • Schedules and Facilitates Quarterly Advisory Committee Meeting.
  • Schedules and Facilitates Yearly Foster Care Provider Meeting.
  • Attends all Mandatory MHMRA HCS Meetings.
  • Coordinates HCS Provider Fair attendance and presentations.
  • Manages MHMRA Provider Fair information videos and brochures.

Education/Experience:

Fifth year college or university program certificate; or two to four years related experience and/or training; or equivalent combination of education and experience.

Computer Skills:

To perform this job successfully, an individual should have knowledge of Word Processing software and Internet software. Ability to work cooperatively with others for integrated programming.

Certificates and Licenses:

Valid Texas driver's license, Non-Violent Crisis Intervention Certification (CPI), CPR/First Aid certified.

Supervisory Responsibilities:

Manages and supervises employees. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

Job Type: Full-time

Pay: $36,000.00 - $46,000.00 per year
