Full-Time Hospice Social Worker (LICSW)

Hospice Social Worker - No Nights, Weekends, or Holidays required!

Hospice of Western & Central Massachusetts provides special care for people who are terminally ill. This involves a team-oriented approach that addresses the medical, physical, social, emotional, and spiritual needs of the patient. Hospice also provides support to the patient’s family or caregiver. Services provided at Hospice of Western & Central Massachusetts mainly includes home health aide, counseling, spiritual services, medical social services, medical supply services, nursing services, and physician services.

Our staff is committed to working closely with the patient, family and caregiver to assist them in understanding the end-of-life process, identifying what they can expect at the end of life, and empowering them to provide care when our team is not present. We are a community-based hospice, providing end-of-life comfort care to patients in any setting that is considered home as well as acute care hospital setting.

The primary purpose of this position is to plan, organize, develop, and direct all required social services to meet the needs our patients and their families in accordance with federal, state, and local standards, guidelines and regulations, that govern our agency, and as may be directed by the Clinical Director to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.

Job Requirements

Degree in Social Work and Massachusetts State Licensure at LICSW level.

Strong background in mental health/counseling and experience in health services delivery.

Ability to work with other members of the Interdisciplinary Team.

Ability to conduct assessment of patient/family psychosocial history.

Ability to identify and prioritize problems with appropriate goal formulation and conduct required interventions.
