Downtown Brooklyn Nursing and rehabilitation centers located in Brooklyn NY are actively seeking Licensed Practical Nurse (LPN) to join our dynamic team of professionals!

Shifts -

7am-3pm

3pm-11pm

Duties and Responsibilities for a Licensed Practical Nurse (LPN) include but are not limited to the following:

Accepts responsibility for own practice.

· Performs nursing care in relatively structured, routine situations. Administers multiple forms/methods of treatment, under the direction of the RN Supervisor

· Prepares residents for diagnostic tests, treatments, examination, PT/OT, etc., according to protocols.

· Sets- up required documentation for physician visits and transcribes orders timely.

· Prepares and administers medications, according to protocols.

· Follow facility procedures /protocols for admitting residents, transferring residents to hospitals/clinics/outpatient and completion of appropriate documentation based on established guidelines.

· Administers intravenous infusion in accordance with the facility’s Nursing Department Policies, procedures and Infection Control Guidelines.

· Reports pertinent observations, treatments, medications, incidents and accidents associated with residents, to the Registered Nurse, and records same as per Nursing Department standards for documentation.

· Receives, reports, and counts controlled drugs with the outgoing shift. Gives report and count of controlled drugs to the incoming shift.

· Makes rounds on all resident rooms, and ensure that the Nursing Assistants under her supervision receive their resident reports, has their assignments and break time scheduled.

· Responsible for daily monitoring of Electronic Medical Record’s dashboard (including “alerts”, incomplete UDAs, Immunization entries, Medication/Treatment alerts, etc.) and addressing any issues identified.

· Ensures new admissions, discharges and transfers are entered in the Electronic Medical Record (Quick ADT) as soon as it occurred.

· Contributes to, and updates nursing care plans in collaboration with other members of the professional health care team.

· Reinforces resident/significant others, teaching programs.

· Assist with rehabilitations programs of resident according to plan of care.

· Provides for physical comfort and safety of residents.

· Contributes to resident care conferences.

· Participates in physician rounds and the discussion of residents’ problems, progress and plans.

· Assists in emergency resuscitative procedures by initiating a “Code Stat” call, performing resuscitative procedures and carrying out other such activities as directed.

Qualifications of a Licensed Practical Nurse (LPN):

· At least one year Long Term Care or hospital experience in resident/patient care services.

· Individual must be able to service residents with a wide range of disabilities, including those with critical and complex diagnosis.

· Must be able to read, write, and speak English.

· Must meet sensory requirements of speech, touch, vision, and hearing to normal with corrections as applicable.

· Must have effective communication skills, both verbal and written.

· Must be creative effective and have good observation and analytical skills.

Education Requirements for a Registered Nurse (RN):

· Graduate of an accredited Licensed Practical Nurse Program
