You could be the one who changes everything for our 26 million members as a clinical professional on our Medical Management/Health Services team. Centene is a diversified, national organization offering competitive benefits including a fresh perspective on workplace flexibility.Responsible for providing housing expertise and support to health plan staff and contracted providers statewide. Responsible for developing a robust housing education and training program to be delivered to health plan staff, service coordinators, and appropriate providers. Works with housing providers to strengthen housing placement and retention efforts for members. Responsible for developing innovative housing programs to fill gaps in the housing continuum as they relate to PHW participants, including older adults and people with disabilities. This is a remote position, but residency within the state of Pennsylvania (specifically Philadelphia) is preferred.
Implement innovative housing programs to help participants remain active members of their community
Assist Service Coordinators, health plan staff, and providers to understand housing options
Track and obtain available housing resources and implement the processes necessary to assist members with securing and maintaining housing and related services
Develop and track metrics to validate program value to PHW and PHW participants
Act as a resource for Service Coordinators when they are experiencing barriers in assisting their participants in obtaining housing
Education/Experience: Bachelor’s degree in Health Care, Human Services, Community Organizing or related field or equivalent experience. 4+ years experience in housing services and understanding affordable housing systems, especially how they relate to people with low income, older adults and people with disabilities. Knowledge of fair housing, tenant's rights, and reasonable accommodations/modifications. Must have a valid Drivers License.
