Position Summary:

Ensures physical and electronic documentation is organized, legible, properly identified/itemized, and in the correct patient’s chart and accounted for according to Medicaid/Medicare and organizational guidelines to facilitate payment for services. Maintains confidentiality and privacy as required by UTH and government regulatory compliance agencies.

Position Key Accountabilities:

1. Locates and retrieves records to accurately audit and verify patient information.
2. Updates client charts in the electronic health record with information in a timely manner.
3. Tracks location of any charts supplemental to the primary electronic health record using appropriate processes.
4. Responds to information requests either in person, email, within the electronic health record, or by telephone in a timely manner.
5. Maintains compliance with UTH and government privacy rules.
6. As requested, locate and make ready charts or information from charts in a timely manner.
7. Participates in privacy monitoring processes
8. Assist with identifying and correcting inaccuracies including duplicate accounts within the patient record.
9. Performs other duties as assigned.

Certification/Skills:

Knowledge of Electronic Health Records software.
Microsoft Office Suite (Outlook, Excel, Word, PowerPoint, Teams etc.)

Minimum Education:

High school diploma.

Minimum Experience:

None.

Physical Requirements:

Exerts up to 20 pounds of force occasionally and/or up to 10 pounds frequently and/or a negligible amount constantly to move objects.
