The Mobile Health Care Coordinator creates charts for Mobile Health program, assists with registration for follow up services if needed. Assists with patient calls and referrals. During non-testing events, this individual provides community outreach to re-engage or respond to individuals needing additional or repeat services. Mobile health program patients of Valle del Sol are served at community-based events at a variety of locations as assigned. This role requires flexibility in scheduling including early hours, late hours, and occasional weekend events. This individual is part of an independent Mobile Health team and helping coordinate event set up, tear down, and follow up is expected.

SKILLS & QUALIFICATIONS:

  • Bilingual
  • 18 years of age or older; and
  • High School diploma and 4 years of experience in a BH setting (1 year must be direct patient contact).
  • Clinical Intern in Bachelor’s or Master’s degree behavioral health program in good standing; or
  • Associate’s degree AND 2 years of experience in a BH setting (1 year must be direct patient contact)
  • Bachelor’s or Master’s degree in behavioral health (B.H.) related field; or
  • Bachelor’s or Master’s degree in a non-BH related field AND 1 year of experience in a BH setting
  • Knowledge necessary to provide the behavioral health services that the agency is authorized to provide
  • Must be credentialed as a BHT II within 90 days of hire
  • Must have strong verbal and written communication skills
  • Culturally sensitive
  • Knowledge of Windows environment and/or Electronic Health Records systems
  • TB test
  • Must have or be able to obtain a valid Fingerprint Clearance Card
  • Must possess, reliable transportation, valid driver's license, and proof of auto insurance
  • Must maintain current CPR/1st Aid certification

Job Type: Full-time

Pay: $19.50 per hour
