Position Summary
The National Palliative Care Supervisor is responsible for modeling the 3 Company values of Compassion, Integrity, and Excellence, and for promoting the Compassus philosophy, using the 6 Pillars of success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. S/he will provide assistance to program leadership in data management to include establishment of tools for data capture and on-going maintenance of data. The National Palliative Care Supervisor will communicate to all palliative care programs as required. S/he is responsible for providing operational leadership, direction, and support to teammates for the Palliative Care service line.
Position Specific Responsibilities
- Provides operational leadership, direction, and support to the Palliative Care Coordinators (PCCs), nurse practitioners, and other teammates for the Palliative Care service line.
- Navigates the dotted lines to lead and direct the PCCs effectively while maintaining excellent communication with their direct managers.
- Assesses for and identifies business inefficiencies and formulates solutions to address concerns proactively.
- Remains a subject matter expert (SME) for the electronic medical record functionality.
- Creates and troubleshoots yearly patient Roster/Scorecard and billing spreadsheets.
- Assists leadership with program metrics.
- Coordinates required metric data and identifies data that falls outside of normal parameters.
- Ensures MIPs/MACRA data is organized and accurate within specified
- deadlines.
- Assists with MIPS/MACRA submission yearly.
- Resolves administrative and operational issues including computer systems, and installed software applications.
- Supervises day to day tactical issues, minimizes customer escalations, and comes to resolutions.
- Leads and manages designated area of Palliative Care to meet department objectives, including people, workloads and work processes.
- Initiates process improvement opportunities proactively, improving efficiencies, and strengthening customer loyalty.
- Creates education for the Palliative Care Coordinator roles utilizing a variety of learning resources.
- Executes leadership/management strategies that are effective, supports palliative objectives and creates a strong, customer-focused organization.
- Handles the first line escalation of operational issues and escalates appropriately to the Director.
- Keeps leadership aware of issues and opportunities to improve efficiencies and customer experiences.
- Develops, implements, and monitors policies and procedures related to referrals for both external or internal operations.
- Ensures complete documentation of all patient referral activities in the population management software for tracking purposes.
- Ensures that staff follows appropriate protocols to comply with department standards.
- Establishes standards of performance and effectively leads staff to meet expectations.
- Oversees the development, analysis, and reporting of referral statistics on a monthly basis or as directed to leadership to meet care coordination standards.
- Works on continuous quality improvement for departments that are assigned which may include responding to patient/provider/staff feedback.
- Attends and actively participates in required meetings and committees as requested.
- Assumes responsibility for continuous improvement activities proactively and is receptive to accepting other job duties as assigned and as delegated.
- Proactively and systematically approaches problems from the mindset of standardizing work across the organization while being mindful of impacts others and to the entire system by holding staff accountable for work performance.
- Leads, develops, and participates in continuous improvement and initiatives, including monitoring and using data, monthly reports, and mandatory reporting data to improve staff clinical processes and quality of care.
- Assists national leadership as requested.
- Performs other duties as assigned.
Education and/or Experience
- Bachelor's degree required.
- At least one (1) year of hospice or palliative care experience preferred.
- Minimum one (1) year of experience in a leadership role in healthcare preferred.
- Experience with using Home Care Home Base EMR preferred.
Skills
- Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
- Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, colleagues, investors, and external parties. Strong written and verbal communications.
- Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces hospice philosophy. Effective communications with physicians, nurses, and other healthcare personnel. Experienced in answering telephones and responding to patients or families. Knowledge of palliative care and symptom control in terminal illnesses. Knowledge of the holistic approach toward patient care as it applies to hospice patients and families. Knowledge of Medicare and commercial insurances. High integrity, including maintenance of confidential information such as patient records.
