GENERAL OVERVIEW:

The Psychiatric Nurse Practitioner is an integral part of the HOPE Clinic, a co-located, multidisciplinary clinical program caring for pregnant, postpartum, and parenting women with SUD, their partners, and their infants from conception of pregnancy through early childhood. The primary responsibility of the Psychiatric Nurse Practitioner is to provide clinical and case management support as part of a collaborative care team with a recovery coach, social work, medical assistant, and obstetrical, psychiatric, addiction, family medicine and pediatric providers to ensure that patient care is coordinated.

The Psychiatric Nurse Practitioner develops, implements and evaluates a program of specialized health care for the patients serviced including performing diagnostic evaluations and medication management. In addition, the primary responsibilities include providing compassionate care to those women, partners and infants while also providing clinical support as part of a collaborative care team with a recovery coach, social work, medical assistant, and obstetrical, psychiatric, addiction, family medicine and pediatric providers.


PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Assesses, diagnoses, and treats psychiatric concerns for adult patients including pregnant and post-partum patients at the HOPE Clinic
  • Provides outpatient pharmacotherapy for addiction, utilizing pharmacotherapies appropriate to clinical needs of the patient, including opioid antagonist or agonist therapy such as naltrexone, buprenorphine.
  • Performs medically delegated aspects of patient care as agreed upon by nursing and medical administrators according to practice guidelines such as ordering and interpreting lab and diagnostic testing, prescribes medications and ordering therapeutic treatments.
  • Will be responsible for follow-ups and therefore will have ongoing contact with the patient (in-person or phone), until the patient is stabilized on medication.
  • Works with a multidisciplinary team of providers to provide expert consultation and to help devise an individualized plan for optimal medical and psychosocial treatment for individual patients and their families.
  • Is willing and able to engage with patients across the spectrum of substance use disorders.
  • Serves as a patient and family advocate and facilitates ethical and patient-centered decision making within the care teams.
  • Collaborates with and develops strong, effective working relationships within MGH including Pediatric group practice, Yawkey obstetrics, infectious disease, psychiatry and addiction medicine, as well as, select primary care sites, agencies and community resources inside and outside of the hospital to develop and implement level of care needs for patients other relevant to facilitate disposition solutions for patients and families.
  • Documents in the medical record according to departmental policies and procedures.
  • Adheres to federal and state policies and ensure appropriate releases are obtained to exchange and disclosure necessary patient information.
  • Work with multidisciplinary team to improve clinic workflow and best practices, including working with the Ambulatory Nurse Manager to ensure compliance with broader clinical policies and procedures.

SKILLS/ABILITIES/COMPETENCIES REQUIRED:

  • Understanding of/experience with supporting psychiatrically complex patients.
  • Completes biopsychosocial and psychopharmacological evaluations
  • Develops treatment plan based on assessment and provides appropriate treatment
  • Maintains working relationship with patient and/or family while maintaining appropriate professional boundaries
  • Maintains effective working relationships with collaborative leadership teams and interdisciplinary health care teams and appropriate community providers
  • Documents in the medical record according to departmental policies and procedures


LICENSES, CERTIFICATIONS, and/or REGISTRATIONS:

  • Current licensure as a Nurse Practitioner or Clinical Nurse Specialist in the Commonwealth of Massachusetts
  • Buprenorphine Waiver required (or willingness to acquire waiver in first 90 days).
  • Current federal and state-controlled substance certification/registration or eligibility.
  • Certified Addictions Registered Nurse - Advanced Practice (CARN-AP) (preferred)
  • EDUCATION: Specify minimum education and clearly indicate if preferred or required

  • Masters in Nursing Required
