About Go Group Program:

The program calendar is dynamic and includes opportunities for social/recreational activities; personal connections; volunteerism; advocacy and self-expression; health and wellness; and independent living skills, among many other opportunities.

This is a leadership role to promote and protect the rights of participants to live a fulfilling life with dignity and respect. We are seeking compassionate leaders who would take our participants on a journey of self-discovery and explore community connections.

Essential Functions & Duties:

  • Plans activities for the program that are meaningful and engaging to the participants.
  • Trains and supports Level II staff in providing participant support, training, and supervision in the areas of communication and mobility.
  • To ensure that participants have access to assistive technology, communication devices, adaptive equipment, in a healthy and safe practices.
  • Provide training of staff in medication administration and ensures that proper medication procedures are followed.
  • Oversees team’s community activities, events and destinations and arranging/accessing transportation.
  • Attends outings with participants.
  • Receives Staff-in-Charge training, including Workers’ Compensation Procedures, Sexual Harassment Prevention, Personnel Policies, Procedures, & Information, and performs duties as required of Staff-in-Charge, including health & safety officer duties, special incident reporting, and instructing and training Level I/II direct service staff in providing participant support.
  • Trained and skilled to maintain participant database (Case Magic), permanent files, and records.
  • Knowledge of all aspects of participant files.
  • Experienced in developing Individual Service Plans (ISP’s), including the supports, activities, and resources required for participants to achieve personal goals, completes daily charting and/or case noting.
  • Evaluates and summarizes participant progress in written form for annual and semi-annual meetings

Qualities of a Community Support Leader:

  • Be a positive role model not only to the participants but to team members as well.
  • Have a collaborative work style using teamwork skills, professionalism, with tact, diplomacy, and good judgement.
  • Possess good organizational and record-keeping skills.
  • Able to prioritize work to meet deadlines, work independently and have some flexibility to adapt to changes.
  • Open to work in the evenings and weekends for participant support, meetings, and community events – if needed.
  • Ability to analyze problems, situations, and alternatives to make effective decisions.
  • Excellent interpersonal and customer services skills.

Job Requirements:

  • At least 18 years of age.
  • Authorized to work in U.S.
  • High school diploma
  • Four (4) years or more of related work experience, preferably with a human service agency or educational organization working with persons with intellectual/developmental disabilities, will be considered in lieu of the educational requirements.
  • To drive participants in a company vehicle (if available) or personal vehicle (mileage will be paid), must possess a valid CA driver’s license, clear DMV record and provide evidence of current auto insurance.
  • Knowledgeable of computers and software use, MS Office (Word, Excel, Outlook) and G Suite Workplace.
  • LiveScan criminal record clearance (or exemption) prior to start.
  • Health Screening and TB Test clearance. Not more than 1 year prior to employment or within 7 days after employment.
  • Physical stamina and ability to meet the physical requirements necessary to perform the assigned duties safely and effectively.
  • Covid Safety Protocols: Aligned with CDC protocols, Futures-Explored requires all employees be fully vaccinated and when eligible; get a booster for COVID-19. In the event of a legal exemption from vaccination, a reasonable accommodation will be considered.

Great if you have:

  • Knowledgeable of Lanterman Act regulations, Regional Center guidelines, and State of California Licensing regulations.
  • Certified in First Aid/CPR/AED by a qualified instructor.
