***Must have experience coordinating 30+ employees and creating schedules without assistance.
The Program Manager supports the Executive Director and assists in group and individual sessions with individuals as determined by their needs.
A&K Homes is looking for compassionate and caring people to join our team of professional in promoting quality of life to individuals in their home.
Program Manager is responsible for overseeing a consumer caseload of as many as 30 individuals and providing program management in accordance with Community Homes for Individuals with Intellectual Disability Regulations, Chapter 6400.
Job Duties Include:
- Reports to the Executive Director
- Attends meetings and in-services as required.
- Adhere and comply with the school policies, rules, regulations and procedures as indicated in the Policy and Procedures Manuals
- Responsible for coordinating the training of the direct service workers in the content of health and safety needs relevant to each individual.
- Responsible for health, safety and welfare of all individuals.
- Responsible for ensuring all fire safety provisions outlined in the regulations.
- Responsible for assuring all training requirements of each direct service worker are met.
- Participates as a member of Committees, including but not limited to: the management team, Emergency Preparedness, Risk Management and Quality Assurance.
- Coordinating a completing assessment.
- Responsible for ensuring compliance with all regulation outlined in Chapter 6400
- Participate in the development of the ISP, including annual updates and revisions of the ISP
- Attend ISP meetings
- Fulling the role of plan lead, as application, annual update and revision of the ISP and ISP review and revisions.
- Reviewing the ISP, annual updates and revisions for content accuracy.
- Reporting content discrepancies to the Supports Coordinators, as applicable, and plan team members.
- Implementing the ISP as written.
- Supervising, signing and dating the monthly documentation of an individual's participation and progress toward outcomes.
- Reporting a change related to the individuals needs to the Supports Coordinator, as applicable, and plan team members.
- Reviewing the ISP with the individual as require under Regulation 6400
- Documenting the review of the ISP as required under Regulation 6400
- Provide the documentation of the ISP review to the Supports Coordinator, as applicable, and plan team members.
- Informing plan team members of the option to decline the ISP review documentation as required under the Regulations.
- Recommending a revision to a service or outcome in the ISP as provided under the Regulations.
- Coordinating the services provided to the individual.
- Coordinating the support services for the family.
- Coordinating the training of the family in the content of health and safety needs relevant to each individual. Also core requirements outlined in the regulations.
- Developing and implementing provider services as required under the Regulations
- Required to attend and participate in program concerts, plays, and special events, some of which may be held on Saturday or Sunday.
- Performs other related duties as assigned by management
- Fill in for DSP shifts for callouts and helping with new tranistions.
Qualifications:
- A master's degree or above from an accredited college or university and one (1) year work experience working directly with persons with disabilities.
- A bachelor's degree from an accredited college or university and two (2) years work experience working directly with persons with disabilities.
- An associate's degree or 60 credit hours from an accredited college or university and four (4) years work experience working directly with persons with disabilities.
- Computer skills required: Microsoft Office
- Certificates, Licenses, Registrations: Pennsylvania State Police background check, Pennsylvania Child Abuse Clearance, Cogent (FBI) fingerprint scan and report, Recognizing and Reporting Child Abuse, Driver's License, 10 Year Employment Related Record

- Some night and weekend work may be required
- Available to support staff via phone 4pm -11pm
- May need to work some Holidays if weekday
