Social Worker - Geropsychiatric Unit (Direct Hire)
**$5k Sign on Bonus and Relocation Assistance available with 3 year commitment**
We are currently seeking a highly qualified Social Worker for an Acute Care Hospital with Emergency Department in Elkhart, KS.
Pay up to $21.00/hr DOE Salary - $43, 680/Year | Shift: Position is Monday thru Friday with hours 8: 00am to 5: 00pm.
Social Worker Duties:
Provides direct social service and counseling to patients/families
Assists in locating appropriate resources for post-discharge care
Participate in multidisciplinary team to coordinate the client' s plan
Cross-trains with team members to serve as back up resource
Assesses and identifies cases for intervention
Prioritizes goals and develops plans based on client needs
Monitors and evaluates outcome of client' s plan
Education:
Bachelors degree or Masters degree required
Licensure/certification/registration:
Current licensure by Kansas State Board of Social Work
LMSW, LCSW or LPC preferred
BLS issued by the American Heart Association
