Edgerton Care Center (ECC) is looking for a self-motivated individual who has a passion for helping the elderly to join our team!
As the Social Worker at ECC, you will be a part of the management team and an advocate for our residents that reside at our facility. You will work closely with the nursing and therapy departments to ensure safe resident discharges and discharge planning.
You will schedule and lead care conferences to ensure resident and family questions and concerns are addressed in conjunction with the interdisciplinary team. You will be responsible for the grievance program and will work closely with the nursing home administrator to ensure compliance.
You will be an active participant in care plans and ensuring resident psychosocial needs are being met. As the social worker you will assess residents and ensure care plans are created and individualized for the residents in our care. You will assist residents with financial and legal matters (Advanced Directives, Applying for Medicaid, referrals to funeral homes, referrals to APS, etc.).

We are looking someone who meets the following requirements:
- Minimum of a Bachelor's degree from a school accredited in the Council of Social Work education
- Licensed by the Board of Social Work in Wisconsin, which must be in good standing
- Knowledge of applicable laws and regulations
- Knowledge of community resources
- Must be able to read, write and communicate in English
- Preferred experience in Long-Term Care

License/Certification:
- Licensed Certified Social Worker (Preferred)
