Position Summary:

Interacts with physicians, staff, patient/family and outside agencies. Direct patient care support functions are provided to neonatal, pediatric, adolescent, adult, geriatric, and frail elderly patients. Assesses, plans, implements, coordinates and evaluates options and services to meet the patient’s needs through evaluation of available resources to provide quality, cost effective outcomes.

  • Provides direct social service and counseling to patients/families
  • Assists in locating appropriate resources for post-discharge care
  • Participate in multidisciplinary team to coordinate the client’s plan
  • Cross train sufficiently to team member’s duties to be able to serve as back up resource
  • Assesses and identifies cases for intervention
  • Prioritize goals and develops a plan based on client needs
  • Monitor and evaluate outcome of client’s plan
  • Proactive case identification
  • Demonstrate behavior supportive of team achieving department goals
  • Actions demonstrate protection of patient privacy and compliance with HIPPA regulations
  • Collaborate with third party payors internal case management/disease management departments as appropriate for discharge planning needs
  • Documentation: Follow all standards of documentation and document in appropriate systems.
  • Other duties as assigned.


Licensure/ Certification/Registration:

  • Current Texas Licensure as a Social Worker required;
  • Current Healthcare Provider BLS issued by the American Heart Association required;
  • Master's Degree and above in Social Work required.
  • CMSW, LCSW or LCP preferred.


Education:

  • Master’s Degree and above in Social Work is required.

Experience:

  • One-year of recent social work experience preferred (ACUTE CARE EXPERIENCE REQUIRED)
  • NEEDS TO HAVE HOSPITAL DISCHARGE PLANNING EXP

Special Notes:


  • Ability to work at a fast pace and to prioritize multiple assignments/projects and respond to numerous requests; ability to resolve conflicts among staff and to work collaboratively with department director, physicians and other members of the healthcare team; ability to ensure operational efficiency; ability to problem solve and make decisions; ability to exercise self-control and tolerate stress when dealing with multiple requests and/or conflicting demands from multiple customers.


  • Needs minimal sustained direction in assessing needs, directing staff and carrying out departmental and professional responsibilities; self-starting and self-motivating; working hours may exceed eight (8) hours per day and is based on what is needed to accomplish work at hand.


Compensation: Min: $25.16- Mid: $31.45 (10 years) -Max: $37.73 (20 years) (all depends on candidates years of experience)
