Social Worker
John Knox Village is looking for a caring Social Worker to work in our Skilled Nursing Facility
POSITION RESPONSIBILITIES (Outcomes)
Obtain and/or review advance directives for each guest at move-in.- Visit the elders and guests to provide clinical services, including provision of clinical care, assessment, reassessment, and evaluation of treatment or interventions.
- Know each elder and his/her family, including a psychosocial profile of the elder and an understanding of the family members and dynamics.
- Assess each elder for unmet needs and monitor the care plan for compliance with social services issues.
- Act as a referral source to elders and family members and document referrals.
- Attend weekly Utilization Review Meetings.
- Safe Transition. Begin Safe Transition planning as soon as possible after admission. Meet with guest/family to review prior level and potential needs at move-out (discharge). Participate in Safe Transition planning, compile and update plans and complete the process. Transportation arrangements as needed. Follow up with discharged guests to evaluate care received and assure Safe Transition to the next level of care.
- Arrange for prescriptions, durable medical equipment, follow-up appointments and other services needed upon discharge.
- Provide information to guests and family members in regards to Medicare, Medicaid, and other social assistance/insurance programs.
- Respond to and facilitate resolution of elder/family problems and concerns within 24 hours of awareness of problem.
- Facilitate family support groups as directed.
- Participate in Quality Assurance projects as directed.
- Complete MDS sections C, D, E, Q for all new guests. Formulate Care Plans in areas of Cognition, Psycho-Social, Mood, Behavior, and Safe Transition, review prior care plans and attend care conferences.
- Complete all necessary documentation for Social Service Department including Assessments, Progress Notes, Care Area Assessments, MDS, Care Plans
- Orient new guests and family members to The Woodlands and the 2-story health center building.
- Maintain regular progress notes indicating response to the treatment plan and adjustment to The Woodlands and the 2-story health center building.
- Visit residents and guests and perform needed services for them, such as: communication with family or friends, utilizing community resources, and informing guests and sponsors of personal and property rights.
- Performs all other business related duties as assigned.
POSITION SKILL REQUIREMENTS (Competencies)
Technical skills and Knowledge
- Bachelor's degree in Social Work or related field.
- 2+ years experience in Social Work, or related field.
- Demonstrated competence with basic computer skills, including but not limited to e-mail, electronic health records, SKYPE and Microsoft Office programs.
- Must possess the ability to identify the social and emotional needs of geriatric residents and guests.
- Must be able to interview, organize and coordinate.
- Demonstrate ability to identify and provide direct services to residents with medical and/or psychosocial disorders.
Interpersonal Skills
- Highly proficient in effectively reading, writing and speaking English.
- Utilize appropriate verbal, writing, listening, and non-verbal skills during open dialogue with elders, family members, clinical support team (occupational, physical, and speech therapists, social workers, dietitians, and activity personnel/recreational therapists) fellow team members and others throughout the community.
- Must have a patient and outgoing disposition.
