• Ensure timely and quality care to members regarding health benefit questions and issues
• Audit and analyze team workflows and results and recommend improvements to management
• Review, investigate and resolve issues from internal and external customers
• Maintain inventory of member materials for events and home visits
• Identifying high risk members and collaborate with case management to strategize outreach to these high utilizers
• Shadow new Member Connections Representatives on home visits and audit results to ensure accuracy and adherence to standards
• Attend various Member Connections related calls and clinical rounds
• Conduct home visits and assist in organizing and staffing member events

Education/Experience: Associate’s degree in related field or equivalent experience. Bilingual ability preferred. 3+ years of member services, social services, or community outreach experience. Previous experience as a lead in a functional area or managing cross functional teams on large scale projects.

License/Certification: Valid driver’s license and proof of insurance.

The salary range for NY is: $61,800 - $111,400

This information reflects the anticipated base salary range for this position based on current national data. Minimums and maximums may vary based on location. Actual pay will be adjusted based on an individual's skills, experience, education, and other job-related factors permitted by law.
