Duties include but not limited to: (Direct Support is a part of the job)

• Oversee relationship building with Community and Families Acting as liaison
• Practice and implement “Person Centered” Thinking, Skills and procedures
• Work with Program Managers to ensure coverage for all clients
• Work with (HR), by being the go-to person for call outs and coordination of replacement staff
• Maintain direct support for a specific client base and Cover shifts when staff coverage is unavailable
• Responsible for providing training via shadowing to new direct support staff
• Responsible for monthly safety checks and effective reporting of all SLS homes
• Responsible for the coordination of individual’s medication with health care providers and efficiently documenting and updating in Case Magic
• Provide back up for the Program Manager during vacations, FMLA, sick, etc..
• Work with Program Managers to address housing needs including RSVP renewals for all individuals
• Review StratusTime daily, follow up with staff and report missed punches and make needed corrections, thus ensuring timely reconciliation of time cards
• Consistently exercises discretion and judgment to analyze, interpret, make deductions and then decide what actions are necessary based on the varying facts and circumstances of each individual case.
• Acquires and provides to the team the detailed information regarding clients to establish the foundation for the individualized service plan including initial screening and assessment.
• Partner with Program manager of individual’s planning team in the development of the individualized goals and objectives using a person centered approach
• Provide data for enhancing and updating the individualized service plans
