We are currently hiring a Clinical Forms Coordinator based in Worcester, MA. The Clinical Forms Coordinator will be responsible for the tracking and processing of inbound forms received in the Medical Department.

Job Summary:

  • Receives and tracks inbound patient related forms. Forms may be received in any number of ways, including via fax, mail or dropped off by patients.
  • Analyzes requirements of forms; notates form to assist in the efficient completion.
  • Prepares forms for review by clinical teams.
  • Consults with provider team as necessary to determine necessary action on forms.
  • Scans and files completed documentation to Medical Records for inclusion in the patient’s medical record
  • Communicate with providers as needed to ensure accurate and complete documentation included on all forms and other paperwork.
  • Acts as a liaison with patients and outside sources with regard to form completion and status.
  • Works extensively with protected health information and is required to adhere to HIPAA privacy and security regulations and policies related to the same.
  • Documents all necessary information regarding form tracking in the PHI log in the electronic health record. This includes all conversations with patients regarding form status and completion
  • Assists the Provider Team with requests for Prior Authorization for medications.

Job Requirements:

  • High School Diploma or GED required
  • Minimum 1 year related experience in a Healthcare environment
  • Computer literacy and Data Entry Skills
  • Ability to navigate and retrieve data from the Electronic Health Record
  • Knowledge of Clinical Documentation
  • Certified Medical Assistant
