Part-Time Case Manager - Assisted Living

We’re hiring a part-time Case Manager to support our assisted living residents in collaboration with our interdisciplinary care team.

As a Part-Time Case Manager, your important role is to brighten the days of our assisted living residents through your own efforts and by effectively coordinating efforts with our care team and support staff. You will provide case management services for our assisted living and enhanced assisted living residents in conjunction with family and/or resident representatives, health providers and other service agencies significant to residents’ needs.

Overlooking beautiful Cayuga Lake, Longview offers independent and assisted living on a lovely 34-acre campus on South Hill. Qualifications

· Experience working with seniors, ideally in assisted living, skilled nursing, home health care, or another setting regulated by NYSDOH

· Can assist full-time case manager in interpreting medical evaluations and assessments

· Have knowledge about insurance and social security

· Degree from an accredited college or university with a concentration in nursing, social work, human services, or gerontology

· Strong organizational skills and ability to work in a self-directed manner

· Excellent written and verbal communication

· Ability to interact in a positive, professional, and supportive manner with residents, families, staff members, students and volunteers

· Ability to maintain timely and accurate documentation

· Sound judgement and excellent problem-solving and decision-making abilities

· Basic computer proficiency

· COVID vaccinations and 1 booster
